Troubleshooting Unpaid Claims

Before submitting a formal inquiry, run through this quick "Audit Checklist" to identify the most frequent roadblocks to payment:

  • Diagnosis Entry: Check the client's chart to ensure a valid ICD-10 diagnosis code is present.

  • Chart Origin: Verify the chart was created by the CCS Benefits Team (Insurance) rather than manually (Self-Pay). If a clinician sets up a chart, they are logging sessions as self-pay and CCS would not see that clinician is logging services.

  • Credentialing: Confirm your "In-Network" effective date for that specific payer.

  • Coding Accuracy: Ensure necessary modifiers (e.g., for telehealth or specific licensure) were applied during logging.

If any of the above factors apply, please submit a Claims Inquiry Form to our billing team to identify the error made and request assistance in rectifying it. If you have checked all of these factors and none apply, please submit the same form to our billing team for further investigation.
